Tirzepatide is a 39-amino acid synthetic peptide that acts as a dual agonist at both glucose-dependent insulinotropic polypeptide (GIP) and glucagon-like peptide-1 (GLP-1) receptors. Developed by Eli Lilly, it received FDA approval in 2022 (Mounjaro) for type 2 diabetes and in 2023 (Zepbound) for chronic weight management.

The molecule is built on a GIP receptor-binding scaffold with GLP-1 receptor agonist activity incorporated via sequence modification, enabling simultaneous activation of both incretin receptors. The SURPASS clinical trial program demonstrated superior HbA1c reduction and body weight loss versus semaglutide across T2D populations. SURMOUNT trials established efficacy in obesity cohorts.
